We are currently accepting pre-enrolments for patients who live within our enrolment area.

 

Enrolling with a general practice gives you access to funded healthcare services, including lower consultation fees and subsidised prescription medicines where applicable.

 

Please note that you can only be enrolled with one general practice at a time. If you are currently enrolled with another practice, your enrolment will only be transferred to Takapau Health on 5th October 2026 when we open (assuming it is approved).

Who can enrol?

​

To enrol, you must be eligible for publicly funded health services in New Zealand.

​

You may be eligible if you are:

• A New Zealand citizen

• A New Zealand permanent resident

• The holder of an eligible work visa or residence visa

• Otherwise eligible under Health New Zealand enrolment criteria

If you are unsure whether you are eligible, please contact our team for assistance before completing your enrolment.

Documents you may need to complete pre-enrolment

​

You will be asked to upload/provide evidence of your eligibility.

​

Depending on your circumstances, this may include:

• New Zealand Drivers License

• New Zealand or International passport; or

• New Zealand birth certificate together with photo identification

• Residence visa or residence permit documentation

• Current work visa documentation

• Birth certificate (children)

 

Additional documents may be requested if required to confirm eligibility.

The information you provide during enrolment is protected by the Privacy Act 2020 and the Health Information Privacy Code 2020. Your personal information will only be used for purposes related to your healthcare and enrolment.
